Graceful Journey: A Workshop about Making End of Life Decisions

Discussions about significant and sensitive issues such as dying can sometimes be emotional and even daunting.  Yet, when a person’s wishes and preferences are known before there is a medical crisis, it can lessen anxiety, pain, conflict and guilt for family members.

Join us at St. Stan’s to learn and practice how to start having these discussions with our families and how to complete a healthcare directive.

Pat Tommet, an Advance Care Planning Facilitator, retired nurse, and member of St. Stan’s Church will conduct these workshops.  Topics covered will include:

  • Experiences with death and making healthcare decisions for other
  • Envisioning your own end-of-life journey (wishes, hopes and concerns)
  • Common treatment choices that often need to be made
  • Barriers to starting the discussion with family and strategies for overcoming these barriers
  • Selecting a healthcare agent
  • Preparing a healthcare directive

This two-part Evening Workshop on Advance Care Planning will be offered:

  • Monday, October 7:  Starting the Discussion – 7:00 to 8:30 PM
  • Monday, November 4:  Completing a Healthcare Directive – 7:00 to 8:30 PM

This two-part Morning Workshop on Advance Care Planning will be offered:

  • Tuesday, October 8:  Starting the Discussion – 10:00 to 11:30 AM
  • Tuesday, November 5:  Completing a Health Care Directive – 10:00 to 11:30 PM

Pre-registration is preferred.  Please contact Kirsten Frederick at kirsten@ststans.org or 651-224-7998 to indicate your preference.  You may also register at the door.
